Self-Pay & Payment Options

For clients choosing private pay, I accept:

  • Cash

  • HSA

  • FSA

  • All major credit cards

Superbills are available upon request for clients using out-of-network benefits.

I am currently in-network with the following Blue Cross Blue Shield PPO plans:

  • Blue Traditional

  • Blue Advantage PPO

  • Blue Choice PPO

  • Blue Preferred PPO

  • Native Blue

  • MyBlue

I am not in-network with any HMO, Medicare, Medicaid, or Medicare Advantage plans.

Coverage varies by employer group and individual policy.

Clients are responsible for contacting their insurance company prior to scheduling to verify:

  • That I am in-network with their specific policy

  • That their plan is a PPO (not HMO, EPO, Medicare, Medicaid, or other plan type)

  • Their deductible, copay, and/or coinsurance amounts

If you carry more than one type of active insurance coverage (such as primary and secondary insurance), I am unable to provide services, as coordination of benefits is not supported in my practice.
